About the Role
We are currently looking to hire a Registered Practical Nurse to join our growing team. We are looking for a driven and passionate individual who is looking for a collaborative approach to client care.
This individual must be able to work well under pressure and perform well in crisis situations.
Candidate must be flexible with schedule as this position may include weekends.
Duties/Responsibilities:
• Operates professionally, ethically and with integrity within the guidelines and standards of the Ontario College of Nurses and adheres to the highest standards of medical practice
• Responsible for completing initial and ongoing medical assessments
• Dispense and administer medications (including injections) as delegated by a licensed physician
• Ongoing collaboration and communication with Physician and Psychiatrist regarding clients wellbeing
• Book appointments with Physician and Psychiatrist for all new admissions, medication refills and discharges
• Collaborate with members of the multi-disciplinary team to plan, implement, coordinate, and evaluate the appropriate treatment plans to support the client.
• Update, maintain and ensure accuracy of patient information within Electronic Medical Records
• Liaise with pharmacy partners to ensure collaborative care and seamless service delivery
• Accompany clients to offsite scheduled medical appointments as required
• Facilitate wellness related groups as required
• Assessing the physical health of all new intakes which may include blood pressure, temperature, injections and medication reviews.
• Attend to client illnesses and wound management and documenting incident reports when necessary.
• Drive clients to medical appointments or pharmacy when needed.
• Monitor and maintain all medication and medical supply stock
• Attend and be an active participant in team meetings, team training and other internal team meetings etc.
• Adhere to all policies and procedures as implemented by the company
• Any other duties as required by management
Reports to: Operations Manager
Requirements
Completion of a Diploma in Registered Practical Nursing
Previous experience in addiction treatment and/or a similar field (Asset)
A member in good standing with the College of Nurses in Ontario.
Current First Aid and CPR Certification is required.
